What is the ''Incident Report'' in Australian Childcare Settings?

In childcare settings in Australia, any incident involving a child must be reported and communicated to the family. This may range from a very minor scratch to more serious injuries that require emergency medical attention. All incidents must be documented in detail and the records must be retained.

Every childcare service has incident report forms. Educators are required to record the details of the incident on these forms. The form typically includes the child’s personal information, the date and time of the incident, the location where it occurred, a description of how the incident happened, and the actions taken afterwards. It also records whether emergency services were contacted and the time at which the family was notified.

When the family arrives to collect the child, they are asked to review and sign the form acknowledging the information provided about the incident. These forms are then stored in individual files created for each child.

Depending on the seriousness of the incident, it is the responsibility of the Director or Nominated Supervisor to notify the relevant department.
